Why is it important to submit my forms ahead of time?
Completing your forms at least 48 hours before your appointment allows our medical team: Nurse Sunshine and Dr. de la Cruz to thoroughly review your health history, medications, allergies, and treatment goals. This helps us identify any contraindications, ensure your safety, and create a customized plan tailored just for you.
What happens if I don’t complete my forms in time?
If your forms are not submitted at least 48 hours in advance, we may need to reschedule your appointment. Our goal is to deliver safe, high-quality care and we can’t do that without having your full medical background reviewed ahead of time
I’m a returning patient, do I still need to fill them out?
Yes. If it’s been a while since your last visit or if you're receiving a new treatment, we require updated forms to reflect any changes in your health, medications, or concerns.
How do I complete the forms?
Once your appointment is booked, you’ll receive a secure link via text or email to complete your forms online. They’re easy to fill out and should take just a few minutes.
