Wellness Program Terms & Conditions

Arawan MedSpa + Wellness

This policy applies to all wellness services, including:

• Appetite Management
• Hormone Optimization
• Vitamin Support
• Any future physician-directed wellness therapies

1. How Our Programs Work

You are not paying for medication alone.

You are paying for:

• Physician evaluation
• Ongoing monitoring
• Dose adjustments when appropriate
• Clinical guidance and support

Your results depend on your consistency and participation.

2. Required Steps to Start Care

Step 1: Consultation (Required)

You must complete a physician consultation before starting any program.

Step 2: Laboratory Testing & Analysis (Required for Ongoing Care)

Laboratory testing is required to safely start and continue treatment.

Lab Pricing

• Hormone Optimization (HRT) Labs:
  • $600 per panel (standard pricing)
  • $360 for Ageless Alliance Program and Heroes
• Standard Wellness Labs:
  • $200 per test

What to Expect

You will need:

• Initial labs before or early in treatment
• Repeat labs during or after your program to monitor progress and safety

Important

• Labs are required for medical decision-making
• If labs are not completed when recommended:
  Care may be delayed or stopped

3. Program Commitment

• Programs require consistency and follow-through
• You must attend follow-ups and follow medical guidance
• Missing visits or not following the plan will affect your results

4. Follow-Up Care 

• Some programs require weekly follow-up check-ins
• These are part of your care and not optional

If you miss check-ins:

• Your progress may be affected
• Your treatment may be adjusted or paused

5. Missed Appointments

• 24-hour notice is required to cancel or reschedule
• Late cancellation or no-show = $200 fee charged to your card on file
• Missed appointments are counted as used program or account credit

6. Payments and Financial Responsibility

• All services are self-pay (no insurance)
• All program payments are non-refundable
• You are responsible for the full program cost once started

Payments continue until the program is completed:

• Even if you miss visits
• Even if you travel
• Even if you pause or stop participating

7. Late Payments

• Late or declined payments = $50 fee per late payment
• Services will be paused until paid
• After 120 days unpaid, account may go to collections

8. Credit Card on File

A valid credit card is required to:

• Secure appointments
• Process payments
• Charge missed appointment fees

9. No Refunds / No Exceptions

• No refunds for unused treatments
• No refunds for missed visits
• No refunds for stopping early

10. Missed Doses / Missed Care

• Missed visits or missed program treatments are considered used
• They are not replaced, refunded, or rescheduled

11. Medication Responsibility (If Applicable)

• You are responsible for storing medications correctly
• Lost, damaged, or improperly stored medications will not be replaced

12. Eligibility

• You must be 18 years or older
• You must be medically appropriate based on evaluation
• Not all patients qualify for treatment

13. Compliance

If you do not:

• Follow medical instructions
• Attend visits
• Communicate with the team

Then:

• Your results will be affected
• Your program may be paused or stopped

14. Program Changes or Termination

Arawan may:

• Adjust your treatment plan
• Pause care for safety or non-compliance
• Terminate care if necessary

15. Disputes / Chargebacks

• You must contact us directly for any concerns
• All charges are valid once services are available
• Chargebacks for valid charges are not allowed

16. Final Acknowledgment

By starting care, you agree to:

• Follow the treatment plan
• Complete required labs
• Attend follow-ups
• Remain financially responsible
