Care is delivered under the guidance of Dr. Eric de la Cruz through clinical oversight, ongoing monitoring, and individualized adjustments based on your response over time.

This program is designed to support long-term healthspan through precision, consistency, and medical expertise.

How the Program Works

All patients begin with a physician consultation to determine eligibility and establish a personalized plan of care.

Step 1: Consultation

• Semaglutide consultation only, no blood draw: $200
• Tirzepatide consultation only, no blood draw: $360

Includes:

• Medical evaluation
• Review of history and goals
• Treatment planning

An introductory starter dose may be provided if clinically appropriate.

Step 2: Laboratory Evaluation (If Continuing Care)

Includes:

• Baseline lab assessment
• Clinical safety evaluation
• Data to guide ongoing management

Follow-Up Laboratory Evaluation:

• Repeat laboratory testing may be required to assess treatment response and ensure appropriate progression
• Follow-up labs are performed at the end of a program or as clinically indicated

Laboratory testing is required for safe, physician-directed care and must be completed as recommended to continue treatment.

Step 3: Program Enrollment

Ongoing care is delivered through a structured, physician-directed program to ensure safe, effective, and consistent management.

Pricing reflects weeks of management, NOT THE NUMBER OF MEDICATION DOSES.

Medications are prescribed only after consultation and are not sold independently.

Flexible Plan of Care (As Needed)

For patients who prefer flexibility without long-term commitment

• Semaglutide: $100 per week of management
• Tirzepatide: $150 per week of management

Includes:

• Physician-directed oversight
• Basic clinical monitoring

Important:

• This plan does NOT include dose adjustments (NO increases or tapering)
• Best suited for short-term or maintenance use

12-Week Plan of Care

For patients seeking a guided, progressive approach

• Semaglutide: $1,200 for 12 weeks of management
• Tirzepatide: $1,800 for 12 weeks of management

Includes:

• Dose adjustments based on response and need to taper
• Ongoing physician-directed medication dose adjustments

24-Week Plan of Care

For patients committed to long-term metabolic support

• Semaglutide: $2,160 for 24 weeks of management
• Tirzepatide: $3,000 for 24 weeks of management

Includes:

• Extended clinical management 
• Dose adjustments based on response and need to taper
• Ongoing physician-directed medication adjustments

Ageless Alliance and Heroes Preferred Pricing

• Semaglutide: $360 every 4 weeks of management
• Tirzepatide: $500 every 4 weeks of management

Includes:

Why Tapering May Be Necessary

Tapering is used to support a safe transition off the medication to maintain long-term results.

It may help:

• Reduce rebound appetite
• Support gradual metabolic adjustment
• Maintain behavioral and nutritional changes
• Prevent rapid return of prior eating patterns

Tapering is necessary for patients who:

• Have been on higher doses
• Have achieved significant body composition changes
• Are transitioning off longer-term use
